What is citalopram, and what is it used for?

Citalopram is an antidepressant medication that affects neurotransmitters, the chemicals that nerves within the brain use to communicate with each other. Neurotransmitters are manufactured and released by nerves and then travel and attach to nearby nerves. Thus, neurotransmitters can be thought of as the communication system of the brain. Many experts believe that an imbalance among neurotransmitters is the cause of depression.

Citalopram works by preventing the uptake of one neurotransmitter, serotonin, by nerve cells after it has been released. Since uptake is an important mechanism for removing released neurotransmitters and terminating their actions on adjacent nerves, the reduced uptake caused by citalopram results in more free serotonin in the brain to stimulate nerve cells.

Citalopram is in the class of drugs called selective serotonin reuptake inhibitors (SSRIs), a class that also contains fluoxetine (Prozac), paroxetine (Paxil) and sertraline (Zoloft). it was approved by the FDA in July 1998.

It is approved for treating depression. It is also used off-label for treating:

Serious side effects

Serious side effects from citalopram oral tablets aren’t common, but they can occur. Call your doctor right away if you have serious side effects. Call 911 or your local emergency number if your symptoms feel life threatening or if you think you’re having a medical emergency.

Serious side effects and their symptoms can include:

  • Abnormal or excessive bleeding. Symptoms can include:
    • bleeding or bruising more than normal
    • blood in your urine or stool
    • nosebleeds
  • Low blood sodium levels. Symptoms can include:

How should this medicine be used?

Citalopram comes as a tablet and a solution (liquid) to take by mouth. It is usually taken once a day, in the morning or in the evening, with or without food. Take citalopram at around the same time every day. Follow the directions on your prescription label carefully, and ask your doctor or pharmacist to explain any part you do not understand. Take citalopram exactly as directed. Do not take more or less of it or take it more often than prescribed by your doctor.

Your doctor may start you on a low dose of citalopram and gradually increase your dose, not more often than once a week.

It may take 1 to 4 weeks before you notice the full benefit of citalopram. Continue to take citalopram even if you feel well. Do not stop taking citalopram without talking to your doctor. Your doctor will probably decrease your dose gradually. If you suddenly stop taking citalopram, you may experience withdrawal symptoms such as mood changes, irritability, agitation, dizziness, numbness, tingling or electric shock-like sensations in the hands or feet, anxiety, confusion, headache, tiredness, nausea, sweating, shaking, frenzied or abnormally excited mood, and difficulty falling asleep or staying asleep. Tell your doctor if you experience any of these symptoms while you are decreasing your dose of citalopram or soon after you stop taking citalopram.

What Drugs Interact With Celexa?

Do not take Celexa if you:

  • are allergic to citalopram hydrobromide or escitalopram oxalate or any of the ingredients in Celexa. See the end of this Medication Guide for a complete list of ingredients in Celexa.
  • take a monoamine oxidase inhibitor (MAOI). Ask your healthcare provider or pharmacist if you are not sure if you take an MAOI, including the antibiotic linezolid.
  • Do not take an MAOI within 2 weeks of stopping Celexa unless directed to do so by your physician.
  • Do not start Celexa if you stopped taking an MAOI in the last 2 weeks unless directed to do so by your physician.
  • People who take Celexa close in time to an MAOI may have serious or even life-threatening side effects. Get medical help right away if you have any of these symptoms:
    • high fever
    • uncontrolled muscle spasms
    • stiff muscles
    • rapid changes in heart rate or blood pressure
    • confusion
    • loss of consciousness (pass out)
    • take the antipsychotic medicine pimozide (Orap) because this can cause serious heart problems.
    • have a heart problem including congenital long QT syndrome

Before starting Celexa, tell your healthcare provider if you are taking certain drugs such as:

  • Medicines for heart problems
  • Medicines that lower your potassium or magnesium levels in your body
  • Cimetidine
  • Triptans used to treat migraine headache
  • Medicines used to treat mood, anxiety, psychotic or thought disorders, including tricyclics, lithium, SSRIs, SNRIs, amphetamines, or antipsychotics
  • Tramadol
  • Over-the-counter supplements such as tryptophan or St. John’s Wort

Important considerations for taking citalopram

Keep these considerations in mind if your doctor prescribes citalopram oral tablet for you.

General

  • You can take citalopram with or without food.
  • You can cut or crush the tablet.

Storage

  • Store citalopram tablets at room temperature, 77°F (25°C).
  • The tablets can temporarily be stored at temperatures between 59ºF and 86°F (15ºC and 30°C).
  • Keep this drug away from high temperatures.
  • Don’t store this medication in moist or damp areas, such as bathrooms.
  • Keep the bottle tightly closed.

Refills

A prescription for this medication is refillable. You should not need a new prescription for this medication to be refilled. Your doctor will write the number of refills authorized on your prescription.

Travel

When traveling with your medication:

  • Always carry your medication with you. When flying, never put it into a checked bag. Keep it in your carry-on bag.
  • Don’t worry about airport X-ray machines. They can’t hurt your medication.
  • You may need to show airport staff the pharmacy label for your medication. Always carry the original prescription-labeled box with you.
